Provides coordinated comprehensive health care to patients of all ages in their home. Specific treatment and services rendered are individualized under the direction of the patient's physician. Home health care is available following an illness, injury, surgical procedure or hospitalization. It is also available for patients whose level of wellness has deteriorated in the home setting. Services can be either long or short term. Levels of care may vary from extensive to minimal needs. Services are rendered on an intermittent or part time basis.
Skilled Nursing assessment of all body systems, blood drawing for laboratory tests, wound care, blood sugar monitoring, injections, IV therapy, medication monitoring, and teaching to patient and family.
Physical Therapy provides a licensed therapist in the home for a variety of treatments including but not limited to exercise programs, mobilization, procedures designed to increase motion, modulate pain and restore function, as well as patient/family education
Medical Social Services are provided by a Licensed Medical Social Worker to assist with accessing financial resources for medical care, psychosocial support and counseling.
Occupational Therapy provides a licensed therapist in the home to assist in restoring physical function, exercise, stretching techniques and independence in self care. All treatment is directed toward improving a patient's level of independence and safety within their home environment.
Home Health Aide are Certified Nurses Aides who have received additional training specific to home care. Services provided include bathing, grooming and personal hygiene.

Durable Medical Equipment (DME)
Staff and Services: Pondera Medical Center's Durable Medical Equipment (DME) staff is professionally trained, certified and/or licensed. A 24-hour Emergency On-Call service is provided for your comfort and peace of mind. Direct billing of Medicare, Medicaid and private insurance companies is available. Pondera Medical Center's DME department is community based and employs local citizens to serve your local needs. PMC's DME department is dedicated to improving your quality of life.
Oxygen and Respiratory Services: Following a physician's order, DME's oxygen/respiratory staff provides the client with a complete orientation of in-home respiratory services, equipment set-up and proper operating procedures.
DME Oxygen/ Respiratory Staff:
- Assess oxygen client's condition on a monthly basis, and provide client's physician with a written report.
- Provide monthly maintenance, testing and cleaning of all oxygen equipment.
- Delivery and set-up of portable oxygen tanks as needed.

- Oxygen Concentrators
- Portable Oxygen Cylinders
- Nebulizers
- Continuous Positive Airway Pressure (CPAP) systems and supplies
- Bi-Level Positive Airway Pressure (BiPAP) systems and supplies
Rehab Equipment and Services: Pondera Medical Center's DME department offers a wide selection of rehab equipment and accessories to meet the client's needs. The DME staff works directly with the client's physician and/or therapists to insure that the most suitable equipment is ordered.
