PONDERA HOME HEALTH AGENCY |
Provides coordinated comprehensive health care to patients of all ages in their home. Specific treatment and services rendered are individualized under the direction of the patient's physician. Home health care is available following an illness, injury, surgical procedure or hospitalization. It is also available for patients whose level of wellness has deteriorated in the home setting. Services can be either long or short term. Levels of care may vary from extensive to minimal needs. Services are rendered on an intermittent or part time basis. Skilled Nursing assessment of all body systems, blood drawing for laboratory tests, wound care, blood sugar monitoring, injections, IV therapy, medication monitoring, and teaching to patient and family. Physical Therapy provides a licensed therapist in the home for a variety of treatments including but not limited to exercise programs, mobilization, procedures designed to increase motion, modulate pain and restore function, as well as patient/family education Medical Social Services are provided by a Licensed Medical Social Worker to assist with accessing financial resources for medical care, psychosocial support and counseling. Occupational Therapy provides a licensed therapist in the home to assist in restoring physical function, exercise, stretching techniques and independence in self care. All treatment is directed toward improving a patient's level of independence and safety within their home environment. Home Health Aide are Certified Nurses Aides who have received additional training specific to home care. Services provided include bathing, grooming and personal hygiene. |
